Opportunity4Kids provides Scholarships for extracurricular activities. Applications for any out-of-school time activities will be considered.

Opportunity4Kids provides funds for fees, uniforms, and equipment to participate in extracurricular activities.

We do not provide funds for transportation or travel costs associated with the activity. All funds are sent directly to the activity provider or uniform/equipment retailer.

To qualify for a Scholarship, the child requesting funds must currently be enrolled in K-12 in Arizona. Additionally, your family’s household income must be within 150% of the poverty level for your household size.

• Household Income Verification: Tax returns or three months of pay stubs from both parents/guardians. If one parent/guardian is not working, please provide the most recent tax return. If parents/guardians are not in the same household, both must submit their income documentation, including any child support or custody agreements.

• K-12 Enrollment Verification: The child's most recent report card or other official school documentation, ensuring that both the child's name and the school's name are clearly visible.

• For Parents/Guardians Who Do Not Work: A comprehensive list of all government assistance received, such as SNAP, TANF, SSI, or other relevant programs. Along with these documents, you must provide financial details that demonstrate your ability to cover transportation to and from the activity, as well as any incidental fees that may arise during participation. Please note that AHCCCS documents are not accepted, and all government assistance documents must clearly display start and end/renewal dates.

If only one parent/guardian is filling out the application, the other parent/guardian can submit their documents via email to Scholarship@Opportunity4Kids.org, including the child's name in the subject line.

For fees, Opportunity4Kids will mail a check directly to the activity provider. Equipment/Uniforms will be purchased online and mailed directly to you. No funds will be sent directly to families and we do not offer reimbursements.

No, Scholarship Applications typically take 3-4 weeks to be processed by our Scholarship Committee after Opportunity4Kids has received ALL documents.

Our average Scholarship is about $300 for three months of activity. If your request is above this amount the Scholarship Committee may offer a partial Scholarship to cover fees, uniforms, and/or equipment.
